There is an opportunity to get physicals tonight at CHS for free. These have been in the D51 emails for the last two months.
Fall 2024 REGISTRATION IS NOW OPEN!!!

Attached are instructions and a physical form.
Students must have ALL PAPERWORK & PAYMENT completed prior to the first official practice date.
Fall Sports 2024
Boys Golf - start date August 5
Preferred Deadline is at least 1 week prior to start date.
The earlier the better so we can guarantee they will be cleared for tryouts.
They can register after the deadline. Clearing the student will be on a first come first serve basis when we receive the registration form.
The reason we need them to sign up as soon as possible is so I can check paperwork, physical, and make sure no medical issues that could hold up the approval process (example, a prior concussion requires a doctor clearance note and possibly Return to Play Protocol which can delay things).
To be cleared:
1. Completed registration
2. Updated physical. THEY UPLOAD THEIR OWN PHYSICAL
BOTH SIDES OF THE PHYSICAL MUST BE UPLOADED.
3. Athletic fee paid ($150). The fee will need to be brought to the budget office OR they can pay online MyPaymentsPlus.
Payment options will open July 15.

The Warrior Wellness Center will do physicals, ($20) sometimes the same day. The number is 970-200-1603 to call for an appointment.
